BIOPSIES (GYN & NON-GYN)

  1. Place each specimen in a pre-filled formalin container.

  2. Label each specimen separately with the full last and first name of the patient and the source/site of the specimen.

  3. Place containers with a completed tissue/non-gynecologic (blue) requisition form in an appropriate biohazard transport bag and seal.

CULTURES

  1. Submit specimen in a sterile container or a sterile culturette swab. Call lab if unsure of how to submit.

  2. Label each specimen separately with the full last and first name of the patient and the source/site of the specimen.

  3. Place container with a completed tissue/non-gynecologic (blue) requisition form in an appropriate biohazard transport bag and seal.

  4. Call for courier pick-up; specimens for culture must be picked up the same day.

  5. Specimens such as fluids submitted in a sterile container with no preservative should be refrigerated until pick up. Specimens submitted using a culturette swab should remain at room temperature.

FNA /NEEDLE BIOPSY

  1. Place each specimen in a pre-filled formalin container.

  2. Label each specimen separately with the full last and first name of the patient and the source/site of the specimen.

  3. Place containers with a completed tissue/non-gynecologic (blue) requisition form in an appropriate biohazard transport bag and seal.

FNA/ FLUID

  1. Place each specimen in a pre-filled CytoLyt vial.

  2. Label each specimen separately with the full last and first name of the patient and the source/site of the specimen.

  3. Place containers with a completed tissue/non-gynecologic (blue) requisition form in an appropriate biohazard transport bag and seal.

PROSTATE BIOPSIES

  1. Place each specimen in a pre-filled formalin container.

  2. Label each specimen separately with the full last and first name of the patient and the source/site of the specimen.

  3. Prostate biopsy kits available from Interscope (contains twelve 20 ml. formalin containers in an organized box).

  4. Place containers with a completed tissue/non-gynecologic (blue) requisition form in an appropriate biohazard transport bag and seal.

URINE CYTOLOGY

  1. Place freshly voided urine specimen in a urine container.

  2. Label each specimen separately with the full last and first name of the patient and the source/site of the specimen.

  3. Place containers with a completed tissue/non-gynecologic (blue) requisition form in an appropriate biohazard transport bag and seal.

  4. Place in refrigerator until specimen is picked-up.

UROVYSION

  1. Submit freshly voided urine specimen in a sterile container.

  2. Label each specimen separately with the full last and first name of the patient and the source/site of the specimen.

  3. Call for courier pick-up; specimens for UroVysion must be picked-up the same day.

  4. Place containers with a completed tissue/non-gynecologic (blue) requisition form in an appropriate biohazard transport bag and seal.

  5. Place in refrigerator until specimen is picked up.

CALCULUS—(STONES)

  1. No Fixative.

  2. Place in a dry container.

  3. Label each specimen separately with the full last and first name of the patient and the source/site of the specimen.

  4. Place containers with a completed tissue/non-gynecologic (blue) requisition form in an appropriate biohazard transport bag and seal.
